PLEASE NOTE:

  1. All members are entitled to receive all notices and publications sent out by the College, and to attend and speak at general meetings but only Full Members and Fellows can vote at general meetings.

  2. The objectives of the Hong Kong College of Family Physicians are:

    1. to provide continuing education to medical practitioners;

    2. to co-ordinate with medical schools and other institutions in the vocational training for family medicine; and

    3. to advance the recognition of family medicine as a discipline in itself.

  3. Please indicate in details the following if applicable:

    1. Previous criminal record or current criminal investigation in any jurisdiction.

    2. Previous medical council disciplinary record or equivalent in any jurisdiction.

    3. Current medical disciplinary investigation or proceedings in any jurisdiction.

  4. Criteria to define active and dormant membership for full members and fellows:

    1. Active Full Member must have a minimum of 30 credit points or more accredited by the credit points scheme of the Quality Assurance & Accreditation. Full Members who fail to achieve this will be Dormant Full Members.

    2. Active Fellows must obtain the Certificate of Quality Assurance to maintain their active Fellowship. With the exception of Honorary Fellows, all other Fellows who fail to obtain the Certificate of Quality Assurance will be Dormant Fellows.

  5. The Council shall in its entire discretion determine whether or not a person applying for membership shall be admitted as a member of the College and shall not be obliged to inform a rejected applicant of the reason for his rejection.

THE HONG KONG COLLEGE OF FAMILY PHYSICIANS
MEMBERSHIP FEE OF HKCFP

(Effective from January 2003)

1. ENTRANCE FEE
Entrance fees shall be payable by the different categories of members as follows:
i. Upon admission as an Affiliate Member HK$300.00
ii. Upon admission as an Associate Member HK$850.00
iii. No entrance fee shall be payable by an Honorary Fellow, Foundation Fellow, Fellow, Non-HKSAR Fellow, Full Member, Non-HKSAR Member and Student Member.
iv. No entrance fee shall be payable by a Student Member who was admitted to become an Associate Member by the Council upon graduation from any local medical school.
v. An Associate Member who was recommended by the Membership Committee to become Full Member shall not be required to pay a further entrance fee.
2. ANNUAL SUBSCRIPTION
A subscription due and payable on the first day of January in each year, shall be paid by the different categories of members as follows:
(i) Foundation Fellow, Fellow HK$ 1,200.00                
(ii) Full Member HK$ 800.00
(iii) Associate Member HK$ 800.00
(iv) Affiliate Member HK$ 300.00
(v) Student Member HK$ 100.00
(vi) Non-HKSAR Fellow HK$ 750.00
(vii) Non-HKSAR Member HK$ 300.00
(viii) A Member who is aged over 65 and has paid membership fee for a total of at least 25 years shall be exempted from paying annual subscription if:
(a) he submits an application to the Honorary Secretary in the form prescribed by the Council and;
(b) his application is endorsed by the Chairman of the Membership Committee and approved by the members in general meeting.
(ix) No subscription shall be payable by an Honorary Fellow. An Honorary Fellow who pays no subscription should not be entitled to vote in the members' meeting nor hold any executive function in the College.
(x) Any Student Member or fresh medical graduate who was admitted to become an Associate Member by the Council and has submitted a certificate indicating the year of graduation shall be entitled to enjoy a 50% reduction in the annual subscription for the first 3 years after graduation.
i.e. An Associate Member who graduated in 2001, 2002, 2003 shall pay HK$400.00 Annual Subscription for the year 2004.

Such entrance fee and annual subscription may be revised by the College in general meeting from time to time.
